Carmen J. Formella, age 76, of Ottawa, passed away at his home Tuesday morning, May 16, 2017.
Memorial services will be held at a later date. Gladfelter Funeral Home is assisting with arrangements.
Carmen was born June 30, 1940 in Chicago, a son of Joseph and Angeline (Passi) Formella. He married Bonnie Clark February 7, 1959 at St. Gerald’s Church in Oak Lawn. Surviving are his wife, Bonnie; children, Joseph Formella of Plainfield and Gina (Arthur) Bird of Ottawa; grandchildren, Anthony Formella, Amber Taylor, Bobbie Jean Formella, Heather Knupp and David Brasfield. He also leaves a brother, Joseph Formella of Chicago and a sister, Joanne McKendrick of Chicago Ridge. Carmen was preceded in death by his parents, a son, Carmen Formella, a brother, John Formella and a sister, Patricia Tulik.
Carmen was a man with a wonderful sense of humor and was considered quite the jokester. He also loved to gamble.
